A 8 cm Fibroid in Early Pregnancy: A Story of Counselling, Choice and Hope

“Doctor, I am only six weeks pregnant… and I have an 8 cm fibroid. What should I do?”

Those were the words of a young woman who came to me in the very early weeks of her first pregnancy.

She was a primigravida—pregnant for the first time—and understandably, she was frightened. Along with the excitement of seeing her pregnancy on the ultrasound, she had also been told that she had a large uterine fibroid measuring about 8 cm.

For a first-time mother, hearing the words “8 cm fibroid” and “pregnancy” together can be overwhelming.

She had many questions.

Would the fibroid harm the baby?

Could the pregnancy continue normally?

Would she need surgery?

Could the fibroid grow during pregnancy?

Would she be able to carry the pregnancy to term?

The first step was not treatment. It was understanding.

We discussed her ultrasound findings, the location and characteristics of the fibroid, her pregnancy, and the possible implications of having a fibroid during pregnancy.

Fibroids are benign growths arising from the muscular wall of the uterus. Many women with fibroids have completely normal pregnancies. However, the effect of a fibroid during pregnancy can depend on several factors, including its size, location and relationship to the uterine cavity and placenta, as well as how the pregnancy progresses.

An 8 cm fibroid is certainly significant and deserves careful follow-up—but its presence does not automatically mean that a pregnancy cannot continue.

We discussed the possible risks and uncertainties, including pain due to fibroid degeneration, changes in fibroid size, placental problems, fetal growth concerns, malpresentation and the possibility of needing a caesarean delivery depending on the circumstances.

Most importantly, we discussed what we knew—and what we could not predict with certainty.

An informed decision

After understanding the situation, she chose to continue her pregnancy.

It was not a decision based on false reassurance.

It was a decision based on information, counselling, understanding of the potential risks and benefits, and her own wishes.

Our approach was to monitor her pregnancy carefully rather than assume that the presence of a large fibroid would necessarily lead to a poor outcome.

The early weeks passed with regular antenatal care and appropriate ultrasound monitoring.

There were moments of anxiety.

Every scan brought the same question:

“Is my baby okay?”

And thankfully, the pregnancy continued to progress.

One trimester at a time

As the pregnancy advanced, our focus remained on both mother and baby.

We monitored fetal growth and wellbeing and remained alert for complications that can sometimes occur in pregnancies complicated by fibroids.

The fibroid remained an important part of the clinical picture, but it did not become the whole story.

The mother gradually became more confident.

The frightened woman who had first walked into the clinic at six weeks began to enjoy her pregnancy.

She learned that having a fibroid did not mean she had to spend nine months expecting something to go wrong.

It meant that her pregnancy needed appropriate observation, communication and individualized care.

Reaching full term

Weeks turned into months.

The pregnancy continued to progress remarkably well.

And eventually, she reached full term.

The day that had once seemed uncertain at six weeks had finally arrived.

She delivered a healthy baby.

For her, it was more than just the birth of her child.

It was the culmination of months of uncertainty, careful monitoring, counselling and a decision she had made after understanding her situation.

And for us, it was a reminder of something very important in obstetric care:

Sometimes the most important intervention is not a procedure—it is good counselling and informed decision-making.

What can women learn from this story?

A fibroid diagnosed during pregnancy can understandably cause anxiety, particularly when it is large.

But a fibroid does not automatically mean that the pregnancy has to be terminated or that a healthy baby cannot be delivered.

At the same time, every pregnancy with a fibroid is different.

The outcome depends on factors such as:

* Size and location of the fibroid

* Number of fibroids

* Whether the fibroid distorts the uterine cavity

* Its relationship to the placenta

* Maternal symptoms

* Fetal growth and wellbeing

* Development of pregnancy-related complications

* Changes occurring during pregnancy

Therefore, management should be individualized rather than based on the size of the fibroid alone.

The message I would like every mother to remember

When you are told something unexpected during pregnancy, it is natural to feel scared.

But fear should not make the decision for you.

Understand the diagnosis.

Ask questions.

Discuss the possible risks and benefits with your obstetrician.

Understand the uncertainties.

Make an informed decision.

And then take the pregnancy one step at a time.

This young mother’s journey from a 6-week pregnancy with an 8 cm fibroid to a full-term healthy baby is one example of how careful monitoring, appropriate counselling and informed maternal choice can make a meaningful difference.

Every pregnancy is unique, and this story should not be taken as a guarantee of outcome. Women with fibroids in pregnancy should receive individualized medical advice and appropriate antenatal monitoring.

Sometimes, hope does not mean ignoring the risks.

Hope means understanding the risks—and moving forward with informed confidence.
