Amniotic fluid is the clear fluid surrounding your baby inside the womb during pregnancy. It plays an important role in your baby’s growth, development and protection.
During antenatal scans, your doctor may mention the amniotic fluid index (AFI) or single deepest pocket (SDP). If you are told that your fluid is low or high, it is natural to feel worried. However, an abnormal fluid level does not always mean that there is a serious problem. Your doctor will interpret the fluid level along with your baby’s growth, Doppler findings, placenta and overall pregnancy.
What is amniotic fluid?
Amniotic fluid is contained within the amniotic sac around the baby. In early pregnancy, it mainly comes from fluid passing through the placenta. As pregnancy progresses, your baby’s urine becomes an important source of amniotic fluid.

What is high amniotic fluid?
Excessive amniotic fluid is called polyhydramnios.
It may be mild, moderate or severe.

Diabetes during pregnancy
Multiple pregnancy
Certain fetal abnormalities affecting swallowing or gastrointestinal function
Fetal anaemia in selected situations
Certain infections
Sometimes a genetic or other underlying condition
In many cases, especially mild cases, no specific cause is found
What problems can high amniotic fluid cause?
Polyhydramnios can increase the risk of:
Maternal breathlessness or abdominal discomfort
Excessive abdominal enlargement
Preterm contractions or preterm birth
Malpresentation, such as breech or transverse lie
Premature rupture of membranes
Umbilical cord prolapse after the waters break
Placental abruption in certain situations
Increased risk of complications during labour and delivery
Again, mild polyhydramnios is often monitored without major intervention, particularly when the baby and other investigations are normal.
Can drinking more water increase amniotic fluid?
Maternal hydration can sometimes cause a temporary improvement in amniotic fluid measurements, particularly when dehydration contributes to reduced fluid.
However, drinking excessive amounts of water is not a treatment for every case of oligohydramnios.
If low fluid is related to placental problems, leaking membranes or another underlying condition, simply drinking more water will not correct the cause.
Follow your obstetrician’s advice regarding hydration and treatment.
Does low amniotic fluid always mean a C-section?
No.
Low amniotic fluid does not automatically require a Caesarean section.

If an underlying reversible factor is present, the fluid may improve. In other cases, particularly when there is placental insufficiency or membrane leakage, it may remain low.
Your doctor may therefore recommend serial ultrasound monitoring rather than relying on a single measurement.
What should I do if my scan says “low fluid” or “high fluid”?
Don’t panic.
Ask your obstetrician:
How abnormal is the fluid level?
Which measurement was used – AFI or SDP?
Is the baby’s growth normal?
Are the Doppler studies normal?
Is there any possibility of leaking amniotic fluid?
Does the baby require additional monitoring?
When should the next scan or check-up be done?
Does the fluid level affect the timing or mode of delivery?
The answer depends on the complete clinical picture rather than the fluid measurement alone.
When should you contact your doctor urgently?
Seek medical assessment promptly if you experience:
Sudden watery leakage from the vagina
Continuous wetness or suspected leaking of waters
Vaginal bleeding
Regular painful contractions
Severe abdominal pain
Significant reduction or absence of your baby’s movements
Sudden breathlessness or severe abdominal discomfort, particularly if you have been diagnosed with excessive amniotic fluid
Do not wait for your next routine appointment if you are concerned about reduced fetal movements or possible rupture of membranes.
